Information the form collects
The inquiry form asks for information when you choose to submit it.
- Full name and email address
- Phone number, organization, and country, when you choose to provide them
- Inquiry topic and message
- Your confirmation that the foundation may use the information to respond
How inquiry information is used
Submitted information is used to review your inquiry, understand its topic, and respond through the contact details you provide.
An inquiry is not published on the website. Submitting one does not confirm a program, partnership, funding arrangement, or place in training.
How the form stores information
After server-side validation, the form sends the inquiry to the foundation's configured Supabase database. Database credentials remain on the server.
If storage is unavailable, the submission returns an unavailable result instead of reporting that the inquiry was stored.
How long information is kept
The inquiry system does not currently apply an automatic deletion period to stored records. A verified retention practice must be approved before launch.
Your choices
Phone number, organization, and country are optional. Consent is required before an inquiry can be submitted.
The form is not designed for emergencies, medical records, or confidential patient information. Please do not include that information in your message.
Privacy questions and requests
The foundation has not yet supplied a verified privacy contact. This is a launch blocker. No unverified email address, phone number, or postal address is published here.
